Calcaneal Locking Plate — Sizes and Fixation Concept

Calcaneal fractures challenge fixation twice over. The bone is almost entirely cancellous, so conventional screws find little purchase, and the posterior facet must be held at joint level or the subtalar joint pays for every millimetre of settling. A locking plate answers both points: its screws form a fixed-angle frame that suspends the facet from below, independent of cancellous-bone quality, while the plate itself restores the outer wall and with it the height of the heel.

The three sizes follow the dimensions of the bone rather than the fracture pattern. A small calcaneus accepts the small plate without overhang at the anterior process or the tuberosity; the extra-large plate gives the frame a wider footprint for a big heel. Size and side are chosen by the operating surgeon at templating, along with the approach — limited sinus tarsi or extensile lateral — that the soft tissues allow.

At fixation the plate is laid on the lateral wall of the calcaneus once the facet has been reduced and held provisionally. The locking screws directed beneath the facet are placed first, building the fixed-angle frame that carries the joint surface; the remaining screws fix the plate to the tuberosity and the anterior process, restoring heel height and width. K-wire holes allow the plate to be tacked in position while alignment is confirmed under the image intensifier. The reduction sequence, the approach and the screw plan are determined by the operating surgeon.
