Midfoot, Talar Neck and Universal Plates — Types and Data

Fusion plating in the foot pursues a single goal: rigid, compressed apposition of two joint surfaces until they unite. A plate across a tarsometatarsal or intertarsal joint holds the prepared surfaces together and resists the bending that walking applies to the midfoot, while compression screws pull the joint tight. The same principle serves the small joints of the hindfoot, and the universal straight plates cover the joints for which no dedicated contour exists.

The talar neck plate answers a different problem. The talar neck carries blood supply into the talar body and sits under thin skin on the dorsum of the foot, so fixation there asks for a small, precisely shaped implant that holds the fracture without crowding the corridor. Within this line the operating surgeon therefore finds both the arthrodesis workhorses and the specialist plate for the talus.

In arthrodesis the joint surfaces are prepared, compressed and then bridged with the plate, which is fixed with the screws of the matching foot system; in talar neck fixation the plate is seated on the dorsal neck once length and alignment are restored. K-wire holes and provisional K-wires hold position while alignment is checked under the image intensifier. The joints to be fused, the reduction and the implant choice are determined by the operating surgeon.
